Municipal Questionnaire

Your community is part of the Southern Middlesex Flood Mitigation and Resiliency Study, and the Study Team needs your help. Flood resilience is the ability of a community to anticipate, prepare for, respond to, and recover from flood events in ways that minimize damage to the built and natural environment, people, and the economy. This includes mitigative measures and resilience strategies to reduce future vulnerability and recovery time. Flood resilience requires careful planning and implementation. This study is designed to develop mitigation and resilience strategies for the study area.   

Please complete the questionnaire below to share information about flooding concerns, infrastructure conditions, and past flood impacts in your municipality. 

Why Your Input Matters 

Local knowledge is essential to understanding where flooding occurs, how it affects infrastructure and neighborhoods, and what conditions may not be reflected in existing data. The information you provide will help the study team evaluate flooding conditions across the study area and identify potential flood resiliency and mitigation strategies. 

To provide the most accurate information, we encourage you to discuss or share this questionnaire with others in your municipality who may have relevant knowledge. This may include your Local Emergency Management Coordinator, Public Works Supervisor, Floodplain Administrator (as identified in your Flood Damage Prevention Ordinance), Building Code Official, Planner, Engineer, Administrator/Mayor, or other relevant staff. A similar survey will also be distributed to residents and business owners.

The survey and flood event table should take approximately 20-30 minutes to complete. Please provide as much information as possible when responding. Completed questionnaires should be returned by May 22nd, 2026. If this information is available in another format, please append any such information to this questionnaire.

We recognize that some of this information may have been provided through the Middlesex County Hazard Mitigation Plan program. Feel free to include that questionnaire response or associated documents to answer some of these questions.
